These buckets of waste include inappropriate care, avoidable hospitalizations, wide variation in care processes and procedures, and administrative inefficiencies. The good news is that all can be corrected by hospitals and health plans. The proven key is a delivery system built with the primary care physician as the focal point of delivery. This is often referred to as the Third Generation of the Patient Centered Medical Home.This is most important to best manage chronic conditions to prevent escalation to more serious consequences.
